Why Scanned Pages Come Out in the Wrong Order
A feeder that scans the fronts and then the flipped backs leaves pages interleaved. Here is the pattern, and the fastest way out of it.

A twenty-page double-sided document goes through the feeder, and the PDF that comes out reads 1, 3, 5, 7 and then 8, 6, 4, 2. Or it reads backwards, last page first. Neither is random, and the pattern tells you which happened.
The interleaved one
A feeder that scans one side at a time takes all the fronts first. You then lift the stack out, turn it over as a block and put it back. That flip reverses the running order, so the backs are scanned last sheet first. The result is every odd page in order, followed by every even page in reverse.
It looks like chaos and is not. Page 1 is where it should be, page 2 is the very last page in the file, and the halves meet in the middle.
The reversed one
Some feeders take the stack face up and pull from the top, sending the last sheet through first. Every page is present and the right way up, and the document counts down. If page 1 is at the end, this is what you have.
The genuinely shuffled one
Double feeds, a jam cleared by hand and a dropped stack produce no pattern at all. No tool can work out an order it cannot see. Here, re-scanning is not a fallback. It is the answer.
The fastest fix is usually the scanner
None of these tools interleaves two halves in one click, and dragging forty thumbnails into place is slower than feeding the stack again. So look at the scanner first: many drivers have a manual duplex option that scans the fronts, prompts you to flip the stack, and merges the halves in the right order itself. It exists for exactly this problem.

Confirm the pattern
Look at the last page in the file. If it is page 2, you have the interleaved case. If it is page 1, the stack simply ran backwards.

Split the halves
For the interleaved case, extract the fronts and the backs as two files. Typed ranges run upwards only, so take 1 to 10 and 11 to 20 as they stand.

Interleave in the preview
Merge the two files and set the order page by page in the preview, which labels each page with its source file. The back half runs last page first, so work it from the bottom up.

Read the printed numbers
Open the result and check the page numbers printed on the pages rather than the thumbnail positions. This is where a single mistake hides.
For a short document, or one where only a few pages are out of place, skip all of that and go straight to the page organizer. Every page appears as a thumbnail, you drag pages into position, and the export matches the grid.
While you are there: the upside-down pages
A duplex scan where every second page is upside down is a different fault with a much easier fix. Rotation is a property recorded on the page rather than a redraw, so it costs nothing in quality, and rotate can target the even pages on their own.
The short version
- Odd pages in order then even pages in reverse means a flipped stack.
- A document that counts down was fed face up, and only needs reversing.
- No pattern at all means re-scanning, because there is nothing to derive.
- Check the printed page numbers on the output, not the thumbnails.
